How did two programs move forward after repeat FDA letters without running a new trial?

The FDA's letters to Replimune and, by Outlook's account, to Outlook said added analyses did not alter its conclusion. Replimune then went to an advisory committee; Outlook appealed to the Office of New Drugs. Both moved forward on trials already in their applications.

Short answer

By different routes, on evidence already submitted. Replimune's application was resubmitted as a Class 1 resubmission, discussed by an advisory committee that voted 10 to 3, and granted accelerated approval on response data from IGNYTE, the single-arm trial behind its original application. Outlook appealed; by its account the Office of New Drugs found that NORSE TWO with confirmatory evidence established substantial evidence and that additional trials were not required. An FDA decision on Outlook's resubmission is not in this record.

What the evidence shows

Both repeat letters said the added material did not change the conclusion. Replimune's, from the FDA's letter itself: the results of the additional exploratory analyses of the RPL-001-16 data do not alter our initial conclusion FDA · Apr 10, 2026 · p. 2. Outlook's, as the company reported its December 2025 letter: the additional mechanistic and natural history data information provided in the BLA resubmission does not alter the previous review conclusion 8-K · Jan 2, 2026.

Both letters pointed toward more evidence. Replimune's: you must conduct and provide the results from adequate and well-controlled clinical trial(s) which demonstrate substantial evidence of effectiveness FDA · Apr 10, 2026 · p. 6. Outlook's, by the company's account, again recommended confirmatory evidence of efficacy 8-K · Jan 2, 2026, and the FDA has not indicated what type of confirmatory evidence would be acceptable 8-K · Jan 2, 2026; a March 2026 filing said it is possible that we may need to conduct additional clinical and/or non-clinical studies of ONS-5010 8-K · Mar 16, 2026.

Replimune's route ran through the review division and an advisory committee. The 10-K: following collaborative communications with the FDA, the Company and the FDA aligned on a path forward for resubmission and reconsideration of the BLA 10-K · Jun 29, 2026; then the FDA accepted the BLA resubmission as a Class 1 resubmission with an action date of August 2, 2026 10-K · Jun 29, 2026. The committee discussed whether the single-arm IGNYTE study, as designed and conducted, allows for a reliable determination of the expected response rate and durability of response in the proposed population 8-K · Jul 31, 2026 and voted 10 to 3 8-K · Jul 31, 2026. The approval 8-K: This indication is approved under accelerated approval based on objective response rate and duration of response. 8-K · Aug 7, 2026 Its efficacy figures come from IGNYTE, the trial the BLA had been based on (the cohort of patients (n=140) in the Phase 2 IGNYTE trial 8-K · Apr 13, 2026): The IGNYTE trial enrolled 140 patients with 91 patients with at least one non-injected lesion included in the efficacy-evaluable population. 8-K · Aug 7, 2026 The second letter had noted that over half (53%) of patients with objective response did not have noninjected target lesions to assess systemic anti-tumor activity FDA · Apr 10, 2026 · p. 4. The confirmatory trial continues: a confirmatory Phase 3 trial, IGNYTE-3, is ongoing 8-K · Aug 7, 2026.

Outlook's route ran above the division. After a formal Type A meeting on March 2, 2026, which led to the submission of the FDRR and associated meeting 8-K · Apr 7, 2026, the company met with the Office of New Drugs at the U.S. Food and Drug Administration (FDA) 8-K · Apr 21, 2026. Its account of the decision: OND determined that the results of the NORSE TWO trial, together with confirmatory evidence including NORSE EIGHT, natural history, and mechanistic and pharmacodynamic data, establish substantial evidence of effectiveness 8-K · May 26, 2026, and therefore additional trials are not required 8-K · Jun 1, 2026. Those were the components of the resubmission the December letter had answered: a comprehensive package of confirmatory evidence, including mechanistic and pharmacodynamic data demonstrating VEGF inhibition and natural history data 8-K · Feb 11, 2026. How did evidence the FDA said did not alter its conclusion come to establish substantial evidence?

The comparison: in both records, a repeat letter that pointed to more evidence was followed by a step forward on evidence already in the application, Replimune's by accelerated approval after an advisory committee vote, Outlook's by the Office of New Drugs on appeal. Replimune's approval rests on IGNYTE response data with a confirmatory trial underway; Outlook's decision, as the company reports it, rests on NORSE TWO with confirmatory evidence. Not in this record: the FDA's approval letter and review for RP1, the Office of New Drugs' decision itself, and any FDA action on Outlook's June 2026 resubmission.

Outlook Therapeutics Form 8-K, filed January 2, 2026

In the CRL, the FDA noted that the additional mechanistic and natural history data information provided in the BLA resubmission does not alter the previous review conclusion that while the one adequate and well-controlled study demonstrated efficacy, the FDA has again recommended that confirmatory evidence of efficacy be submitted to support the application, however the FDA has not indicated what type of confirmatory evidence would be acceptable.

Moreover, on December 31, 2025, we reported that we had received a third Complete Response Letter, or CRL, with respect to our Biologics License Application, or BLA, for ONS-5010, and the U.S. Food and Drug Administration, or FDA, has again recommended that confirmatory evidence of efficacy be submitted to support the application. However, the FDA did not indicate in the CRL what type of confirmatory evidence would be acceptable, and it is possible that we may need to conduct additional clinical and/or non-clinical studies of ONS-5010 in order to satisfy the FDA’s requirements

While a randomized controlled trial was preferred, the FDA suggested in the March 2021 Type B minutes that if the data was sufficiently compelling, a single arm trial could be acceptable for consideration under accelerated approval. At the subsequent pre-BLA meeting, the FDA stated “we do not object to your proposal to submit a BLA based primarily on data from the cohort of patients (n=140) in the Phase 2 IGNYTE trial who had advanced melanoma and progressed while being treated with prior anti-PD-1 based therapy.”
