Jaypirca: what changed in the evidence · Jaypirca · 2023 – 2026

Did each approval build on the last, or rest on a new body of evidence?

Both, by the record. The two 2023 approvals drew on one study; each later approval rested on its own new randomized trial in its own population. The earlier single-arm results stayed in the label alongside the new trials.

Short answer

The record supports both readings in part. The two accelerated approvals came from cohorts of the same BRUIN study. The 2025 conversion rested on a new trial the FDA specified in 2023, in a redefined population; the 2026 approval on another new trial in untreated patients. Each randomized trial answered its own question rather than re-measuring the earlier one, and the single-arm results remain in the current label.

What the evidence shows

One study for both 2023 decisions: Efficacy was evaluated in BRUIN (NCT03740529) FDA · Jan 27, 2023 for mantle cell lymphoma; for CLL/SLL, an open-label, international, single-arm, multicohort trial FDA · Dec 1, 2023 of the same study.

A trial set by the FDA's own requirement: 4557-1 asked for pirtobrutinib or an investigator’s choice arm of idelalisib plus rituximab or bendamustine plus rituximab FDA · Dec 1, 2023 · p. 2, and BRUIN CLL-321 randomized patients to receive either pirtobrutinib or investigator’s choice of idelalisib plus a rituximab product (IR) or bendamustine plus a rituximab product (BR) FDA · Dec 3, 2025.

A trial in a new population: BRUIN CLL-313 enrolled 282 patients with previously untreated CLL/SLL without deletion of chromosome 17p FDA · Oct 2, 2026, patients outside every earlier indication.

What did not carry over: the mantle cell lymphoma trial the FDA required in 2023 has not reported in the FDA's record. Where does the confirmatory trial for Jaypirca's first indication stand? What stayed constant: What stayed the same across Jaypirca's approvals?

4557-1 Complete a randomized trial in patients with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL). Patients should be randomized to receive pirtobrutinib or an investigator’s choice arm of idelalisib plus rituximab or bendamustine plus rituximab. The primary endpoint should be progression-free survival, with secondary endpoints that include response rate and overall survival.

FDA announcement, December 3, 2025

Patients were randomized (1:1) to receive either pirtobrutinib or investigator’s choice of idelalisib plus a rituximab product (IR) or bendamustine plus a rituximab product (BR). Crossover to JAYPIRCA monotherapy was permitted for patients in the investigator’s choice arm after confirmed disease progression.

FDA announcement, October 2, 2026

Efficacy was evaluated in BRUIN CLL-313 (NCT05023980), a randomized, open-label, active-controlled trial in 282 patients with previously untreated CLL/SLL without deletion of chromosome 17p. Patients were randomized 1:1 to receive pirtobrutinib (n=141) until disease progression or bendamustine plus a rituximab product (n=141) for six cycles.
