Jaypirca: what changed in the evidence · Jaypirca · Oct 2026
What evidence supported Jaypirca's approval in previously untreated CLL/SLL?
BRUIN CLL-313: 282 untreated patients without 17p deletion, randomized against bendamustine and rituximab. Progression-free survival hazard ratio 0.20 at a median 28 months; overall survival immature. The approval letter carries no accelerated-approval terms.

Short answer
A second randomized trial, in a different population. BRUIN CLL-313 randomized 282 previously untreated patients without 17p deletion to pirtobrutinib or six cycles of bendamustine and rituximab; at a median follow-up of 28 months the hazard ratio for progression-free survival was 0.20, with the pirtobrutinib median not reached and 33.5 months on the comparator. Overall survival was immature. The October 2, 2026 approval is not an accelerated approval.
What the evidence shows
The letter: This Prior Approval supplemental new drug application provides for a new indication for adult patients with previously untreated chronic lymphocytic leukemia or small lymphocytic lymphoma (CLL/SLL) with no known 17p deletion
FDA · Oct 2, 2026 · p. 1. The FDA's October 2, 2026 approval letter does not mention accelerated approval, a confirmatory trial or a postmarketing requirement. FDA · searched
The trial: Patients were randomized 1:1 to receive pirtobrutinib (n=141) until disease progression or bendamustine plus a rituximab product (n=141) for six cycles
FDA · Oct 2, 2026. The result: With an estimated median follow-up for PFS of 28 months, the median PFS was not estimable (NE) (95% CI: NE, NE) in the pirtobrutinib arm and 33.5 months (95% CI: 32.7, NE) in the bendamustine and rituximab arm (Hazard ratio 0.20 [95% CI: 0.11, 0.37]; p-value <0.0001)
FDA · Oct 2, 2026.
Survival: At the time of the primary PFS analysis, overall survival (OS) data was immature. The median OS was not reached in either arm with a total of 13 deaths: three deaths (2.1%) in the pirtobrutinib arm and 10 deaths (7.1%) in the control arm
FDA · Oct 2, 2026. Crossover: Eighteen out of the 141 patients in the BR arm crossed over and received JAYPIRCA therapy
FDA · Oct 2, 2026 · p. 21. Safety: Serious adverse reactions occurred in 28% of patients taking pirtobrutinib

Lilly reported the result in February 2026 as reducing the risk of progression or death by 80%, versus chemoimmunotherapy in patients with treatment-naïve CLL/SLL
