Jaypirca: what changed in the evidence · Jaypirca · 2023 – 2026

How do Jaypirca's four FDA decisions compare, side by side?

Population, trial, design, comparator, endpoint, result and regulatory basis for each decision, quoted from the FDA. The first two are single-arm and response-based; the last two are randomized against chemoimmunotherapy-based comparators and measure progression-free survival.

Short answer

Two pairs. The 2023 decisions share a study (BRUIN), a design (single-arm), an endpoint (response rate) and a basis (accelerated approval). The 2025 and 2026 decisions each rest on their own randomized, open-label trial against a rituximab-based comparator, with progression-free survival assessed by an independent committee, and neither carries accelerated-approval terms. The table sets out each decision in the FDA's words.

What the evidence shows

Each cell quotes the FDA's announcement, letter or label for that decision. The populations narrow and widen: the two accelerated approvals cover patients after at least two prior lines; the traditional approval covers patients treated with a covalent BTK inhibitor; the 2026 approval covers untreated patients without 17p deletion.

Not in this record: a trial comparing the two randomized studies with each other; they enrolled different populations against different comparators. Whether the evidence accumulated: Did each approval build on the last, or rest on a new body of evidence?

FDA announcement, January 27, 2023

The main efficacy measures were overall response rate (ORR) and duration of response (DOR), as assessed by an independent review committee using Lugano criteria. The ORR was 50% (95% CI: 41, 59) with a complete response rate of 13%. The estimated median DOR was 8.3 months (95% CI: 5.7, NE), and the estimated DOR rate at 6 months was 65.3% (95% CI: 49.8, 77.1).
